LIABILITY WAIVER & ASSUMPTION OF RISK

Participation in dance and movement-based activities involves inherent physical risks.

By enrolling in or participating in any class, rehearsal, workshop, event, or activity at MVMNTUM Dance Center, I acknowledge and agree to the following:

I understand that dance activities, including but not limited to ballet, contemporary, ballroom, mobility, and conditioning, involve physical exertion and risk of injury. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur during participation.

I hereby release, waive, and discharge MVMNTUM Dance Center, its owners, instructors, employees, contractors, and affiliates from any and all claims, demands, or liabilities arising from participation, to the fullest extent permitted by California law.

I affirm that I (or my child) am physically able to participate and have no medical conditions that would limit safe participation. I agree to follow all studio rules, safety guidelines, and instructor direction.

For participants under 18 years of age, the parent or legal guardian assumes full responsibility and consents to participation on behalf of the minor.

In the event of an emergency, I authorize MVMNTUM Dance Center to obtain medical treatment if necessary and accept responsibility for any related costs.
